We are seeking a dedicated Physical Therapist for home health
services in Pueblo, CO, offering flexibility and autonomy. Provide
individualized physical therapy care in patients' homes according
to physician plans. Work as part of an interdisciplinary team,
supporting patients and families. Supervise PT Assistants and
ensure compliance with regulations. Prepare clinical summaries and
participate in discharge planning. Qualifications include
CAPTE-approved education, current licensure, and at least 2 years
of adult PT experience; home health experience preferred. Benefits
