autonomously to deliver physical therapy treatment, either through
referral or direct to consumer. Evaluates patients, develops
treatment plans, assesses patients, and monitors progress toward
treatment goals. Ensures patient needs are met through access to
care, patient experience and quality of care metrics. Assists in
program development and enhancement to meet patient and community
needs. This job description is a summary of the primary duties and

Education Required - Bachelor's degree in physical therapy. Work
Experience Required - None. Certifications Required - Current
physical therapy license in the state of practice OR eligibility
for licensure by examination OR reciprocity from another state.
Basic Life Support (BLS) from the American Heart Association.

The physical essential functions of this job include (but are not
limited to) the following: Frequently exerting 10 to 20 pounds of
force to move objects; occasionally exerting up to 100 pounds of
force. Physical demand requirements are in excess of those for
